Community members are welcome to pick-up service applications at their local Mountain CAP location during normal business hours. *Applications are to be completed offsite
Applicants will need to complete the application in its entirety, gather all required documentation, and then call their local Mountain CAP office to schedule an Intake appointment.
*We ask that applicants have no more than two people present for the Intake process.
